Forest, Oneida, and Vilas County residents are being asked to participate in this year’s Community Health Assessment. County health officials need input on various health related topics to better inform county health programming decisions and aid in the development of a Community Health Plan.

“The survey results will provide important information for healthcare clinicians when it comes to local health policies and programming decision making,” said Sandy Anderson, Regional Vice President and President of Ministry’s Northern Region Hospitals. “Your responses shape our future goals and we really hope everyone in the community finds an opportunity to complete this critical survey.”

Surveys can be found online here and in paper format. Paper copies of the surveys at your local library, churches, chambers of commerce, as well as county and city departments.

The three county health departments, in collaboration with Ministry Saint Mary’s Hospital, Ministry Eagle River Memorial Hospital and Howard Young Medical Center, are working with the University of Wisconsin-Stout’s Applied Research Center to combine efforts and keep expenses low, while collecting as much valid and reliable data as possible. Responses will be anonymous and participants will be given an opportunity to register for drawings for gift certificates from local partners after completing the survey. Responses will be collected through November and once complete, results will be released to the public. For questions about the Community Health Assessment, please contact your local county health department.
