Forms for athletic participation are no available and may be picked up at James Williams Middle School. Enter through main entry and there is a table with all forms just outside the office. Front doors are open on weekdays from 7 a.m. to 1:45 p.m.

Most participation forms may also be accessed via the school district web page.
